I'm no expert but i would Definitely stake to prevent additional damage. I'd try and probably push to get that 60 degree blow at least back 30 degrees...closer to straight. Nice and easy see how it goes. It's very important to stake young fruit trees.

Powdery Mildew? http://www.agf.gov.bc.ca/cropprot/tfipm/mildew.htm Just like Cedar Apple Rust (your other problem), powdery mildew is also a fungus. While i don't have any MAC trees personally, from the quick goggling I did it sounds like another tree is recommended.
